Responsibilities
Work with multidisciplinary teams to align services for clients.
Create and maintain a resource directory for clients and staff.
Assist clients in completing applications for government assistance and benefits.
Document and report on client progress and outcomes.
Requirements
Education
Master's degree in Social Work or a related field is preferred
Experience
Experience in case management or advocacy roles is highly regarded.
