Job Description

As a Case Manager, you will provide crucial support and guidance to clients, assisting them in navigating life's challenges. Your role will involve developing care plans, providing crisis intervention, and making appropriate referrals to further assist individuals in their journey.

Responsibilities

  • Assess clients' needs and create tailored support plans.
  • Coordinate resources and services for clients.
  • Provide ongoing support and advocacy for clients.
  • Maintain detailed documentation of client interactions.
  • Work collaboratively with community organizations for resources.
  • Facilitate family meetings and support groups.
  • Monitor client progress and adjustments to care plans.
  • Stay updated on policies and regulations impacting services.
  • Offer educational workshops for clients and families.
  • Support clients during transitions in services.

Requirements

Education
  • Bachelor's degree in a relevant field such as Social Work or Counseling
Experience
  • A minimum of 2 years of experience in case management, particularly with vulnerable populations
