Clinical Pharmacist

Job Description

We are seeking a dedicated Clinical Pharmacist to join our team in Orange. In this role, you will collaborate with healthcare professionals to ensure optimal medication management and patient care. You will be responsible for conducting medication reviews, providing counselling to patients, and educating healthcare staff on pharmacotherapy.

Responsibilities

• Analyze medication management and identify opportunities for improvement.
• Support the integration of clinical pharmacy services into patient care.
• Assist in clinical audits and ensure adherence to best practices.
• Develop educational materials to enhance patient understanding.
• Facilitate medication reconciliation and safe transitions of care.
• Promote the responsible use of medications within the community.

Requirements

Education

• Doctor of Pharmacy (Pharm.D.)

Experience

• 5 years of experience in a clinical pharmacy setting

• Clinical Assessment

Certifications

• Advanced Practice Pharmacist Certification

Languages

• English: Fluent

Advantageous
