Job Description

As a Social Worker at Community Empowerment Services, you will work closely with individuals and families, helping them navigate through various social challenges. Responsibilities

• Conduct comprehensive assessments for individuals and families.
• Design and implement programs that address social determinants of health.
• Actively participate in team meetings and case conferences.
• Maintain professional relationships with clients and stakeholders.
• Develop resource and referral materials for clients.
• Advocate for policy changes to improve services for clients.
• Document case notes and maintain up-to-date files.
• Provide education and support to clients regarding available resources.

Requirements

Education

• Bachelor's degree in Social Work or related field
• Master's degree in Social Work is highly regarded

Experience

• 2+ years of practical experience in social work

• Behavioral Assessment

Certifications

• Accredited Mental Health Social Worker

Languages

• English: Fluent
