Responsibilities

  • Assess client needs through interviews and observations to provide appropriate support.
  • Create care plans that reflect the individual needs and preferences of clients.
  • Offer direct emotional support to clients during difficult times.
  • Help clients navigate available social services and resources.
  • Advocate for clients, ensuring their voices are heard in decision-making processes.
  • Document all interactions and services rendered confidentially and accurately.
  • Work closely with a multidisciplinary team to enhance service delivery.
  • Conduct regular site visits to ensure clients are living in safe and healthy environments.
  • Evaluate the impact of support services and adjust strategies accordingly.
  • Engage in ongoing training and professional development opportunities.

Requirements

Education
  • Relevant diploma in Social Work or Community Services
  • Bachelor of Social Work is preferred
Experience
  • 2-4 years of experience in community service roles
