Pharmacy Consultant

Job Description

As a Pharmacy Consultant, you will provide expert advice on medication needs to clients while ensuring safe and effective pharmaceutical care. You will work closely with healthcare professionals and contribute to enhancing patient outcomes across our services.

Responsibilities

• Conduct medication reviews and provide recommendations to optimize treatment plans.
• Maintain accurate documentation of all client interactions and medication information.
• Lead educational sessions for healthcare providers on medication management.
• Review and develop medication management plans tailored to patient needs.
• Engage with clients to educate them on proper medication usage and adherence.
• Collaborate with other healthcare providers to enhance patient care.
• Monitor and evaluate client outcomes to ensure effectiveness of medication therapy.
• Assist in the development of pharmacy policies and procedures.
• Provide insights during multidisciplinary team meetings regarding pharmaceutical needs.

Requirements

Education

• Bachelor of Pharmacy
• Advanced degree or certification in pharmacy or healthcare management is preferred

Experience

• 5+ years of experience in a clinical pharmacy setting

Technical Skills

• Knowledge of Electronic Medical Records
• Clinical Pharmacy Skills

Certifications

• Clinical Pharmacy Certification

Languages

• English: Fluent
