Clinical Pharmacist

Job Description

We are seeking a dedicated Clinical Pharmacist to join our team in Bendigo. In this role, you will be responsible for providing expert pharmaceutical care, counselling patients on medication therapy, and collaborating closely with healthcare teams to improve patient outcomes. Your expertise will directly impact the health and wellbeing of our community.

Responsibilities

• Conduct comprehensive medication assessments and reviews.
• Collaborate with physicians to ensure safety and efficacy of treatments.
• Facilitate discharge planning and medication reconciliation.
• Document patient care activities in electronic health records.
• Manage pharmacy inventory and assist with procurement.
• Participate in clinical research and trials as required.
• Engage in community outreach and education on medication safety.
• Promote adherence to clinical guidelines and protocols.

Requirements

Education

• Bachelor's degree in Pharmacy or equivalent
• Postgraduate qualification in Pharmacy Practice is desirable

Experience

• 5+ years of experience in a hospital pharmacy
