Responsibilities

• Work collaboratively with multidisciplinary teams to coordinate client care.
• Facilitate access to health and social services for clients.

• Participate in regular staff meetings and professional development.
• Provide support and consultation to family members of clients as needed.
• Contribute to program development and implementation.

Requirements

Education

• Bachelor's degree in Social Work or related field
• Master's degree in Social Work is preferred

Experience

• 2+ years of experience in a clinical or community setting
