We are seeking a dedicated Healthcare Social Worker to serve as a Discharge Planner. This role is crucial in coordinating the discharge process for patients, collaborating with families, healthcare professionals, and community resources to ensure safe transitions.

Responsibilities

• Conduct comprehensive assessments to identify patients' discharge needs.
• Serve as the primary contact for patients and families during the discharge process.
• Monitor and document discharge plans, ensuring they are updated according to patients' changing needs.
• Provide support to patients in navigating healthcare systems and accessing resources.
• Work collaboratively with multidisciplinary teams to ensure continuity of care.
• Participate in case reviews and contribute to the professional development of team members.
• Advocate for patients' needs and preferences in the discharge planning process.

Requirements

Education

• Master's degree in Social Work or related field preferred
• Licensure or registration with relevant professional body

Experience

• Experience in discharge planning or case management preferred
