Job Description

The Medication Historian plays a crucial role in our healthcare team, responsible for gathering and documenting comprehensive medication histories for patients. This role requires excellent communication skills and attention to detail to ensure patient safety and effective medication management.

Responsibilities

  • Analyze medication use data to identify trends and patterns.
  • Provide recommendations for medication safety improvements.
  • Participate in patient safety and quality audits related to medication management.
  • Advocate for patients’ medication needs and preferences.
  • Educate healthcare providers on best practices for medication reconciliation.

Requirements

Education
  • Additional certifications in medication management or reconciliation are advantageous.
Experience
  • Experience in a clinical pharmacy or related field is preferred.
