Job Description

We are seeking a dedicated Rehabilitation Counselor to act as a Care Navigator for our patients. In this role, you will provide support and assistance to individuals and their families navigating the healthcare system. You will collaborate with healthcare professionals, coordinate care, and empower patients with knowledge about their health conditions.

Responsibilities

• Develop care plans based on individual patient needs in collaboration with healthcare professionals.
• Act as a patient advocate during consultations and treatment planning.
• Provide support in managing healthcare appointments and resources.
• Educate patients about community support services and resources available to them.
• Facilitate communication between patients and healthcare providers to resolve any concerns.
• Maintain up-to-date knowledge about healthcare systems and practices.
• Document all patient interactions and care coordination efforts meticulously.

Requirements

Education

• Bachelor's degree in a healthcare-related field or equivalent experience
• Additional certifications in care coordination are highly valued

Experience

• A minimum of 2 years' experience working in a healthcare setting or patient advocacy role

Technical Skills

• Customer Relationship Management (CRM) software
• Telehealth platforms

Languages

• English: Fluent
