Job Description

We are seeking a compassionate and dedicated Social Worker to join our team in Port Macquarie. The successful candidate will work closely with individuals and families, providing support and intervention services aimed at improving their well-being and quality of life. This role involves conducting assessments, developing care plans, and liaising with community resources to ensure that clients receive the necessary support.

Responsibilities

• Collaborate with healthcare professionals and community agencies to coordinate services.
• Advocate for clients' rights and access to necessary resources.
• Maintain accurate and confidential client records and documentation.
• Facilitate workshops and support groups for diverse populations.
• Monitor client progress and adjust care plans as needed.
• Participate in community outreach initiatives to promote awareness of available services.
• Stay updated with best practices and legislative changes in social work.

Requirements

Education

• Bachelor's degree in Social Work
• Master of Social Work (MSW) is preferred

Experience

• 3+ years of experience working in community service settings
