Job Description

We are seeking a dedicated Registered Nurse Case Manager to join our dynamic healthcare team. You will be responsible for leading care management for patients and helping them navigate their healthcare needs effectively.

Responsibilities

• Manage the development of care goals and interventions for patients.
• Ensure timely communication with healthcare teams involved in patient care.
• Document care coordination activities and patient interactions accurately in electronic medical records.
• Participate in quality improvement initiatives within the organisation.
• Provide crisis management support to patients and caregivers as needed.

Requirements

Education

• Bachelor of Nursing or higher qualification
• Specialisation in Chronic Care Management is desirable

Experience

• A minimum of 3 years’ experience in a clinical nursing role, with a preference for experience in community health.
