Job Description

As a Community Health Worker, you will play a pivotal role in bridging the gap between healthcare services and local communities. Your primary responsibilities will include developing and implementing health promotion programs, conducting educational workshops, and collaborating with various stakeholders to foster community health.

Responsibilities

  • Design and lead health awareness campaigns within the community.
  • Conduct outreach activities to engage underserved populations.
  • Facilitate partnerships with local organizations to promote shared health goals.
  • Assess the effectiveness of programs and suggest improvements.
  • Support community members in navigating health systems and services.
  • Gather and analyze health data to support program development.
  • Prepare and present reports to stakeholders on health initiatives.
  • Promote preventative health measures and healthy lifestyle choices.
  • Assist with grant applications and fundraising efforts.
  • Maintain confidentiality of client information and adhere to privacy regulations.

Requirements

Education
  • Bachelor's degree in Health Promotion or a related field
  • Master's degree in Public Health is preferred

  • 3+ years of relevant community health experience
