Registered Nurse (Home Health RN)

Job Description

As a Home Health RN, you will provide nursing care to patients in their homes, ensuring their physical, emotional, and social well-being. You will collaborate closely with healthcare teams to develop personalized care plans.

Responsibilities

• Deliver high-quality nursing care in patients' homes.
• Evaluate patient health and adapt care plans accordingly.
• Document patient care activities and maintain confidentiality.
• Educate patients on health maintenance and disease prevention.
• Collaborate with a multi-disciplinary team to ensure comprehensive care.
• Perform wound care and management.
• Participate in patient and family meetings to discuss care goals.
• Provide feedback to the management team on patient care quality.
• Help foster a supportive environment for patients and their families.

Requirements

Education

• Bachelor of Science in Nursing (BSN) is preferred

Experience

• Experience in home health care or community nursing is an advantage.
