Job Description

We are seeking a dedicated and compassionate Nephrology Social Worker to join our team. The successful candidate will provide comprehensive social work services to patients diagnosed with kidney disease and undergoing dialysis treatment. This role involves assessing patients' psychosocial needs, developing individual care plans, and advocating for their rights while assisting them in accessing community resources. The ideal candidate will have a strong understanding of the emotional, social, and financial challenges faced by those with kidney conditions and must demonstrate exceptional interpersonal skills.

Responsibilities

  • Evaluate patients' psychosocial needs concerning their kidney health.
  • Work closely with nephrologists and other healthcare professionals to provide holistic care.
  • Engage patients in developing their care plans to address their individual needs.
  • Assist families in navigating the healthcare system and accessing support services.
  • Maintain confidentiality and ethical standards in all patient interactions.
  • Participate in educational initiatives to promote kidney health awareness.
  • Document all interactions accurately for compliance and quality assurance.
  • Use telehealth platforms for remote counseling sessions when appropriate.
  • Stay informed about advancements in kidney disease treatment and management.
  • Provide community resources and referrals to enhance patients' wellbeing.

Requirements

Education
  • Bachelor's degree in Social Work
  • Additional certifications in counseling or mental health are advantageous.
Experience
  • 3+ years of relevant experience in nephrology or renal healthcare.
