Job Description

As an LTTS Service Clinician, you will be pivotal in delivering comprehensive clinical support to individuals needing long-term services. Your responsibilities will be centred around assessing patient needs, developing care plans, and coordinating with a multidisciplinary team. Responsibilities

  • Conduct assessments and evaluations of clients' needs for long-term care.
  • Craft tailored care plans and facilitate access to required services.
  • Provide ongoing support and monitoring of client progress.
  • Coordinate care among multidisciplinary teams, including doctors, nurses, and therapists.
  • Educate patients and their families about available resources and services.
  • Ensure compliance with regulatory and legal guidelines.
  • Document and maintain detailed client records.
  • Participate in health promotion activities and workshops.
  • Engage in professional development opportunities.
  • Collaborate with community agencies and services for comprehensive care.

Requirements

Education
  • Bachelor's degree in Nursing, Social Work, or a related field
Experience
  • Minimum of 3 years of relevant clinical experience, preferably in long-term care or chronic disease management.
