As an LTTS Service Clinician, you will be pivotal in delivering comprehensive clinical support to individuals needing long-term services. Responsibilities

• Conduct assessments and evaluations of clients' needs for long-term care.

• Educate patients and their families about available resources and services.

Requirements

Education

• Bachelor's degree in Nursing, Social Work, or a related field

Experience

• Minimum of 3 years of relevant clinical experience, preferably in long-term care or chronic disease management.
