Job Description

As a Home Health RN, you will provide exceptional nursing care to patients in their homes, aiming to improve their overall health and comfort. Your role will involve assessing patient conditions, administering medications, and coordinating with other healthcare professionals.

Responsibilities

• Evaluate and address patient needs through assessment and observation.
• Deliver emergency care and respond to critical situations.
• Document care provided and assess outcomes of intervention.
• Facilitate patient referrals to specialists as needed.
• Advise patients on lifestyle changes that promote better health.
• Train and support junior nursing staff when necessary.
• Engage in community outreach to promote health education.

Requirements

Education

• Registered Nurse (RN) license in Australia
• Bachelor of Nursing or equivalent qualification preferred

Experience

• Minimum 3 years of nursing experience, with a focus on home care settings.
