Rehabilitation Counselor

Job Description

As a Rehabilitation Counselor, you will play a crucial role in supporting individuals with developmental disabilities. You will assess client needs, develop personalized care plans, and collaborate with a multidisciplinary team to ensure comprehensive service delivery.

Responsibilities

• Lead assessments to evaluate clients' support needs.
• Collaborate with therapy staff to deliver integrated care.
• Professionally advocate for clients' rights and needs.
• Train families on support strategies and available resources.
• Document client progress and review care plans regularly.
• Coordinate case plans with multi-disciplinary teams.
• Participate in community workshops and informational sessions.
• Continuously assess service effectiveness and suggest improvements.
• Stay updated on field developments and best practices.
• Support initiatives that promote client inclusion in the community.

Requirements

Education

• Bachelor's degree in Community Services, Social Work or similar
• Master's preferred but not essential

Experience

• Experience in the disability support sector highly regarded
